Now you’ve learned about our programs, here is what you need to know about aha’s referral process.
Community Supported Residential Units (CSRU)
- WACHS Mental Health Service will assess your referral for clinical case management support
- aha will assess a person’s suitability for the program
Mental Health Housing Program
- Participants must be referred to this program while they are receiving care in a specialist mental health inpatient unit.
Transitional Housing Program
- After referral, aha will assess a person’s suitability for the program
Outreach Program
- After referral, aha will assess a person’s suitability for the program
Community Engagement & Social Inclusion Program
- After referral, aha will assess a person’s suitability for the program
- NDIS participants can be referred to this Program
NDIS
- A NDIS Plan must be in place
- Referral form is required – this can be completed by your NDIS Support Coordinator
