If you want to talk to someone before going to your GP, WACHS Community Mental Health or another mental health professional for a referral, contact us with any questions you might have or to simply talk you through our services or referral process.

For GPs, mental health professionals and other agencies.

Now you’ve learned about our programs, here is what you need to know about aha’s referral process.

Community Supported Residential Units (CSRU)


  • WACHS Mental Health Service will assess your referral for clinical case management support
  • aha will assess a person’s suitability for the program

 

Mental Health Housing Program


  • Participants must be referred to this program while they are receiving care in a specialist mental health inpatient unit.

 

Transitional Housing Program


  • After referral, aha will assess a person’s suitability for the program


Outreach Program


  • After referral, aha will assess a person’s suitability for the program


Community Engagement & Social Inclusion Program


  • After referral, aha will assess a person’s suitability for the program
  • NDIS participants can be referred to this Program


NDIS


  • A NDIS Plan must be in place
  • Referral form is required – this can be completed by your NDIS Support Coordinator
